At The SHAC, there are a variety of forms we may require from you both prior to, and during enrolment.

ENROLMENT PACk
Prior to starting at The SHAC, there are certain documents which you must complete and/or peruse
Below are the instructions for what you need to complete prior to starting
1 – SHAC Family Handbook (Read)
2 – Download KT Connect from Google Play or the APP Store
3 – Check your personal details, bookings, payment details and customer reference numbers (CRN) are correct
4 – Book your orientation time with administration
5 – Medical Conditions Risk Minimisation Plan (To be returned ONLY if child has Medical Condition/Allergy / Anaphylaxis / Asthma)
6 – Extra Curricular Activities and Escort Form (To be returned ONLY if you child is to be taken to / from an extra curricular activity)
